| Jack Chase, 47, of Santa Rosa Beach, Florida was killed in a line of duty death on October 20, 2004. A medical team traveling by helicopter was dispatched to DeFuniak Springs, Florida to transfer a critically ill heart patient to a Pensacola hospital. Shortly after takeoff the pilot radioed back stating the team was turning back because of bad weather. The helicopter was reported missing, search crews found the crash helicopter and its team in the Choctawhatchee Bay. There were no survivors.<br /> <br /> Mr. Chase was a flight nurse for Sacred Heart Health Systems located in Pensacola Florida.<br /> <br /> Honored 2008 |
